The information collected on this form will be used by the “Culte de la Cathédrale Américaine de la Sainte Trinité”, a French law association (the “Cathedral”), solely for the purpose of organizing the participation of your children in its Youth Ministry programs and for informing you of the various activities of the Cathedral. These information will be communicated solely to the Cathedral’s staff and volunteers on a need to know basis. They will be kept for 18 months as from the date of their collection.
